- Engage with members through proactive outreach to motivate, educate, and facilitate access to healthcare programs.
- Conduct comprehensive assessments via phone and/or text to evaluate member needs and develop personalized care plans.
- Manage a dedicated caseload of members, ensuring timely communication and follow-ups to support their care journey.
- Coordinate care by scheduling appointments, collaborating with healthcare providers, and maintaining member engagement with health initiatives.
- Identify and address both social and medical needs impacting members’ well-being.
- Maintain accurate documentation of actions and updates within a care management platform.
- Maintain high outreach volumes to connect effectively with members, families, and providers as necessary.
Required Skills
- Fluent in English; proficiency in Spanish is advantageous.
- Strong interpersonal, verbal, and written communication skills.
- Familiarity with Apple devices (e.g., MacBook) and experience with care management or electronic health record platforms.
- Understanding of motivational interviewing techniques.
Qualifications
- 2+ years of experience in care coordination, case management, community health, pharmacy, or social work.
- Previous experience working with Medicaid and/or Dual Eligible patients is preferred.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) credentials are highly desirable.

Work Schedule
