Job Description:
We are seeking a highly motivated and experienced Certified Medical Assistant (CMA)) to join our team at Medimote. This position is FULLY REMOTE . This is a 1099 contract position for the 1st year and will transfer to W-2 the second year. The CMA will be responsible for enrolling patients in our Chronic Care Management and Remote Patient Monitoring programs, and providing ongoing care management and support to patients with chronic conditions. The CMA will work closely with our provider team and clinical staff to ensure the highest level of patient care.
Qualifications:
Active CMA license
At least 1-2 years of experience in a clinical setting, preferably in chronic care management or remote patient monitoring. New Grads welcomed
Strong communication and interpersonal skills, with the ability to build rapport and trust with patients
Must be friendly and enjoy working with the geriatric population
Knowledge of chronic care management best practices, medication management, and patient education
Able to work independently
Ability to problem-solve
Experience with electronic health record systems such as eclinicalworks, advancedMD, EPIC, etc
Must have liable internet

Ability to work independently as well as in a team environment
Responsibilities:
The CMA will work closely with our provider team and clinical staffs to ensure the highest level of patient care
Provide ongoing care management and support to patients with chronic conditions, including chronic condition education, ability to provide local resources
Conduct regular check-ins with patients viaphone
Monitor patients’ health status and alert providers to any changes or concerns to clinical staff and providers
Must make quality patient care a top priority
Coordinate with clinical staff to ensure patients receive optimal medication management and education
Report any medications management to provider
Write daily notes for patient engagement
Maintain accurate and up-to-date patient records in our electronic health record system
Participate in team meetings and collaborate with other healthcare professionals to ensure high-quality patient care
- Must be efficient in navigating and utilizing web-based electronic health records
- Must be able to communicate with medical providers, patients, and teammates on a professional level

Compensation
- Payment Structure: Compensation will be based on completed CPT codes for Chronic Care Management services. Specifically:
o CPT 99490: Chronic care management services, at least 20 minutes of clinical staff time spent on care coordination within a calendar month.
o CPT 99439: Additional 20 minutes of CCM services, if needed, beyond the initial 20 minutes.
o CPT 99487: Complex CCM services, including 60 minutes of clinical staff time spent on care coordination and additional complexities.

Please note that time logged for less than 20 minutes in a calendar month for a single patient will not be billable, and therefore, will not be compensated. Accurate tracking of time spent on each patient’s care is essential to ensure billing is compliant with Medicare requirements and to guarantee proper payment.
- Starting Salary: $15-20 per hour, with payment based on the number of completed and billable CPT codes.
Contract Details
- Position Type: 1099 Contract
- Starting Salary: $15/hr+
- Probation Period: 90-day probation period. Performance will be reviewed at the end of this period.
- Salary Review: Salary boost upon successful completion of the probation period.
- Bonuses: Eligible for annual bonuses based on performance and achievement of care management goals.
