1. Develop and implement population health strategies to improve health outcomes for members and the community.
2. Collaborate with cross-functional teams to identify and address population health needs.
3. Analyze data and trends to identify opportunities for improvement and develop action plans.
4. Monitor and evaluate the effectiveness of population health programs and initiatives.
5. Develop relationships with community organizations and stakeholders to support population health efforts.
6. Stay updated on industry trends and best practices in population health management.
7. Communicate and collaborate with internal and external stakeholders to drive positive change and achieve population health goals.

8. Utilize project management skills to ensure successful execution of population health initiatives.
9. Advocate for the importance of population health and promote awareness and education within the organization.
10. Assist in the development and management of budgets and resources for population health programs.
11. Provide guidance and support to team members and other colleagues on population health strategies and initiatives.
12. Regularly report on population health performance and progress to leadership.
13. Conduct trainings and presentations on population health topics for both internal and external audiences.
14. Ensure compliance with all relevant regulations and guidelines related to population health.
15. Continuously seek ways to improve and innovate population health efforts to better serve members and the community.

Job Qualifications
• Bachelor’s Or Master’s Degree In Public Health, Healthcare Administration, Or Related Field.
• Minimum Of 5 Years Of Experience In Healthcare Management, Population Health, Or Health Promotion.
• Strong Understanding Of Healthcare Systems And Population Health Principles.
• Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With A Team And Engage Diverse Stakeholders.
• Knowledge Of Data Analysis And Reporting, With The Ability To Develop And Implement Population Health Initiatives Based On Data Insights.
